NHS Bath and North East Somerset, Swindon and Wiltshire (BSW) Integrated Care Board (ICB) is seeking to secure a Provider who has the capability and capacity to deliver an NHS Outreach Health Checks service, in line with the specified requirements. The service will deliver NHS Health Checks to eligible adults aged 40-74 years who have no previously diagnosed cardiovascular disease and who are registered with a GP and/or reside within BSW footprint.
